一、簡介五味子
《神農本草經》記載：五味子「主益氣，咳逆上氣，勞損羸瘦，補不足。強陰，益精。」基原為木蘭科(Magnoliaceae)植物北五味子Schizandra chinensis Baill.及華中五味子(南五味子) Schizandra sphenanthera Rehd. et Will的乾燥成熟果實。經現代植物化學研究，其中所含成分包括有：木質酚類(lignans)、五味子素(schizandrin)、五味子醇(schizandrol)、五味子酯(schisantherin, gomisin)、vitamin C、tannin…等。
二、活性研究
肝炎為台灣常見的疾病，臨床上中醫常以「清利濕熱」及「益氣養陰」為治療原則。當肝臟細胞受到病毒、藥品、酒精、環境中毒性或氧化物質侵害就會引起發炎反應。因此，近年來針對肝臟保健作用，常以抗氧化、抗發炎活性作為肝臟保護之探討指標，整理其中與五味子相關的研究如下：
1. 抗氧化活性：GSH (γ-glutamylcysteinylglycine; Glutathione) 具有捕捉自由基，防止細胞受到氧化傷害的作用，為細胞內重要的抗氧化劑。Ko KM. 等人於1995以CCl4 (carbon tetrachloride)誘導的急性化學性肝損傷實驗中發現，連續服用3天五味子萃取物0.2-3.2 g/kg，能增加 GSH level 與glutathione reductase 活性1。而在以acetaminophen誘發肝毒性研究中亦發現，給予實驗動物gomisin A(50 mg/kg)後，對aminotransferase 活性與脂質過氧化物的形成具有抑制的功效2。
2. 抗發炎活性：在2008年Lian YG.等人，分別以體內及體外實驗探討五味子活性成分抗發炎機轉。在體外試驗方面，以1μM LPS (Lipopolysaccharide) 誘發巨噬細胞(RAW 264.7)的發炎反應，schisandrin (100 μMΜm)能效減少發炎物質NO與PGE2 (prostaglandin E2)的釋放，並抑制COX-2（cyclooxygenase-2）、iNOS (inducible nitric oxide synthase)的表現，效果與indomethacine (50μM)相當。在體內試驗方面，以10 mg/kg LPS誘導小鼠發炎反應的動物模式中，於注射schisandrin (100, 200 mg/kg)與dexamethasone (50 mg/kg)後4小時，schisandrin ( 200 mg/kg)與dexamethasone均能明顯降低血漿中NO的濃度。2010年另一項研究五味子中gomisin N、gomisin J與 schisandrin C成分亦具有類似抗發炎的功效3。
3. 肝臟保護活性：ZHAO WX.等人，於以混合酒精的飼料餵食實驗動物8周後，以LPS誘導肝臟發炎反應，實驗結果發現，於餵食飼料中加入加0.5 %和1.5 %五味子乙醇萃取物能降低血清中ALT的濃度4。
三、結語
「保肝、養肝」為近年保健市場上炒作的議題，號稱取自天然植物成分之產品琳瑯滿目，功效經證實者卻是寥寥無幾。雖五味子對肝臟保護藥理研究尚待加強，與中藥典籍記載「益氣」、「強陰」等傳統功效仍無法結合，且相關臨床研究也不多。但近年衛生署積極推動以藥理研究為基礎的健康食品認證制度，本文應可提供藥師進一步研發五味子相關產品之參考。
